Supporting Families Impacted by Hurricane Helene
Disaster Relief Fund
In light of the devastation caused by Hurricane Helene, Flournoy Cares Foundation and Primecare Home Care Services are coming together to offer immediate financial relief to families who have been displaced or are dealing with significant water damage. We are providing 50 disaster relief grants of $300 each to those in need.

HOW TO APPLY:
Complete the application form below by providing your personal information, a brief description of the damage, and upload a picture of the damage. Please ensure that your Zelle information is accurate, as all funds will be disbursed via Zelle. Applications will be processed on a first-come, first-served basis, and you will be notified once your application has been reviewed.
ELIGIBILITY:

Applicants must have been directly affected by Hurricane Helene.

Applicants must reside in Georgia.

Displacement or significant water damage should be documented with a photo upload.

Only one application per household will be accepted.
